What Miyamoto was likely talking about here is a third "Mario brother", so we’ve updated the post’s title to more clearly reflect that. Right now there are Mario and Luigi as the two main brothers. Miyamoto was probably discussing the possibility of having a third alongside Mario and Luigi.

Luigi is a brother to Mario, meaning Mario is a brother to Luigi, so if they add another brother…he would be a third brother! Eureka!

Am I the only one getting footage from Pokemon Scramble? I don’t see an interview. But it would be interesting to see what a third brother would be. Mario is courageous, Luigi is cowardly, what could the third one be?
